Keeping muscle after fifty: protein plus lifting, not either alone
Protein supplements on their own did nothing for muscle in adults over 50. Alongside lifting, the same protein added about half a kilo of arm and leg muscle and a firmer grip.

Protein powder on its own does almost nothing for an older body. Across 28 trials in adults aged 50 and over, extra protein without exercise left lean mass and grip strength where they started. In the trials where people also lifted, it added 0.54 kg of arm and leg muscle and 1.71 kg on a hand-grip test.1
Why the same steak does less at seventy
Muscle answers two signals. One is the amino acids arriving from a meal. The other is the mechanical load of working a muscle against resistance.
With age the first signal weakens. A given serving of protein sets off less muscle building in an older adult than in a younger one. That is why anyone expected extra protein to help in later life. The trials that tested the idea directly came back inconsistent.1
What protein alone did
The largest analysis restricted to older adults collected 28 trials of protein supplements and higher-protein diets, in people averaging 50 years or more.1 Set against people eating less protein, the extra protein changed nothing measurable. Not total lean mass, not the muscle in the arms and legs, not grip, not knee-extension strength.1
One of those outcomes has a name worth learning once. Appendicular lean mass is the muscle in your arms and legs, the tissue that gets you out of a chair, and it is what most of these trials weigh.
A wider review of healthy adults of every age, 65 studies, found a small effect in the same direction. Protein supplements raised lean body mass by 0.62 kg in adults and 0.46 kg in older adults.2 Grip strength did not move. Leg press strength did not move. The time of day people took the protein made no difference either.2
Whey, the product most often sold for this, has its own analysis in people aged 60 and over: 30 studies.3 Whey did not improve grip strength, gait speed, the six-minute walk, the timed up-and-go test, lean mass or fat mass.3 A single gain showed up, in arm and leg muscle, and it rested on 2 studies.3
What lifting alone did
Exercise has the firmer record. Among 26 trials in older adults who had already lost muscle, exercise by itself raised grip strength by 1.12 kg over usual care.6 It also improved balance, cutting 1.76 seconds off a timed walking test.6 Balance is what decides whether a stumble becomes a fall, and no supplement here moved it.
A larger set of 42 trials, median age 72.9, put resistance training among the most effective things anyone had tested for quality of life, at standardized differences of 0.68 to 1.11 over usual care.7 A standardized difference is a way of putting results from different questionnaires on one scale. On that scale 0.2 counts as small, 0.5 as moderate and 0.8 as large.
Lifting moves strength, balance and function with no supplement involved. The open question is what protein adds on top of it.
What the two did together
In the older-adult trials, the picture changed the moment lifting entered the protocol. Protein plus lifting beat lifting on less protein by 0.54 kg of arm and leg muscle and by 1.71 kg of grip.1 The muscle gain sat almost entirely with the frailest participants, where it reached 0.88 kg. In healthier older adults it was not significant.1
Whey repeated the pattern in miniature. Across the 11 whey trials that included lifting, lower-body strength improved by a standardized 0.25, at the bottom of the small range.3 Whey added nothing to grip, walking or body composition overall.3
Ten trials in women past menopause tell the same story more sharply. With resistance training, whey improved biceps-curl strength by a standardized 0.68 and lower-limb muscle by 1.10.4 Without training it improved neither, and went with a drop in the protein those women ate from food.4
The head-to-head numbers close it. Where exercise alone raised grip by 1.12 kg, exercise with nutrition raised it by 2.03 kg.6 In the 42-trial set, resistance and balance work plus nutrition came out best for grip, at 4.19 kg, against the 5 kg that counts as a change a person would notice.7 A review that gathered up the other reviews landed on the same split verdict: protein with training built more lean mass and more muscle than training alone, and no more strength.5
Where the evidence thins
The gains are real and they are small. Half a kilogram of muscle, spread over four limbs across a few months, will not show in a mirror. It is not what the tub implies.
The trials behind them are also uneven. Most of the 28 older-adult trials carried some risk of bias, and their results scattered widely around the average.1 Nothing in that analysis linked the size of the gain to age, to the protein dose per meal, to how long the trial ran, or to how much protein people already ate.1 So the evidence supports protein alongside lifting, and says nothing about how much protein. The review of reviews rated the underlying quality as moderate to very low.5
Mass and strength also part company here. Protein added to training raised muscle mass more reliably than it raised strength. The review of reviews found no strength advantage at all, and the whey trials found strength only on narrow measures: lower-body strength in one, biceps curl in the other.5,3,4 Strength is what keeps someone climbing stairs, and it is the outcome that answered least.
What remains unknown
Who should do this, and with how much protein, is unsettled. The muscle benefit appeared in frail and sarcopenic participants and not in healthier older adults, so whether a fit sixty-year-old gains anything from extra protein on top of training is still open.1
Duration is the larger gap. The middle trial in the biggest exercise analysis ran 12 weeks.7 Nobody has followed people long enough to say whether the extra half-kilogram is still there years later, or whether it changes who falls, who stays independent, and who does not.

- The trials say no. Across 42 trials in older adults who had already lost muscle, median age 72.9, resistance-based programs improved strength and physical function, and adding nutrition to resistance and balance training raised grip strength by 4.19 kg. The gains were measured over a median of 12 weeks, not years.
- Does protein powder help older adults who do not lift weights?
- Mostly not, on the evidence pooled so far. Without resistance exercise, protein supplements had no significant effect on lean mass or grip strength in adults over 50, and whey alone did not improve handgrip, walking speed or body composition in people over 60. One broader analysis found about 0.46 kg more lean body mass in older adults with supplementation, with no matching gain in strength.
